A 35 year old lady who wished to get pregnant, with irregular period since the age of 30 came to see me. Her menstrual cycles’ length ranges from 45 to 90 days. After a series of ultrasound examinations and blood tests, she is found to be not ovulating. Her fallopian tubes were patent and husband sperm is normal. She was given clomiphene 1 tablet to start with, and then increased to 2 tablets. Eventually she got pregnant successfully.

Clomid

Clomiphene is a milder form of ovulation drug. It has been used in the medical field for 50 years. It is effective to stimulate ovulation and therefore is suitable for women who have irregular ovulation. It is usually prescribed for 5 days, starting on day 2 or 3 or 4 or 5 of menstrual cycle. One to three tablets per day is usually prescribed per day.

Ovulation usually occurs 5-10 days after the last tablet. Ultrasound is very helpful to track ovulation and predicts the best time for sexual intercourse. Sometimes an injection can be given additionally to induce the release of the ovum from the ovarian follicle and optimize the chance of getting pregnant.

Most women will respond to this drug, though some may not. Overweight will reduce responsiveness and weight reduction may be helpful. If you do not response to Clomiphene, and don’t want to use needle for ovulation stimulation, you may add one more drug or try another drug. Some women who do not respond to Clomiphene will respond to combined drug therapy or simply switching to another medication.

Clomiphene is not useful in women who are already ovulating. One should note that women must be tested for tube patency before using this drug. Otherwise you are wasting your valuable time.

Minor side effects are common, such as hot flush and abdominal discomfort. Severe side effects occurs in less than 1/1000 patients.
